Job Description: Population Health Care Coordinator (RN/LPN)

Required Qualifications:
• Graduation from an accredited RN or LPN program recognized by the Oklahoma State Board of Nursing and credentialed with the Oklahoma Board of Nursing
• BLS for Health Care Providers
• Diabetic Education Certification (or willing to obtain within two (2) years of hire)
• Hearing Conservation Certification (or willing to obtain within one (1) year of hire)
• Pulmonary Function Certification (or willing to obtain within one (1) year of hire)
Preferred Qualifications:
• Knowledge of population health programs and resources
• Knowledge of the Health Insurance Portability and Accountability Act (HIPAA)
• Ability to thoroughly interview patients to assess overall health status
• Must be able to educate patients on a variety of health care related issues
• Must be comfortable speaking with patients in-person, over-the-phone, and/or virtually
• Proficient in Microsoft Office, Epic, Cerner, and other necessary applications
• Ability to multi-task and/or switch quickly from task to task
Responsibilities
• Assess, plan, implement, and evaluate educational activities planning for an assigned population.
• Implement new programs based on community health needs and strategic planning discussions.
• Assess patient health problems and needs according to the latest nursing standards, develop and
implement nursing care plans, and maintain accurate and complete medical records (ex. Medicare
Annual Wellness Visits).
• Coordinate necessary health-related services for patients within and outside of this facility.
• Maintain open channels of communication between all appropriate providers, patients, and
administration.
• Administer a variety of examination tests including but not limited to audiometric testing,
pulmonary function testing, sleep study testing, and diabetic examinations, providing education to
patients as necessary.
• Continually evaluate patient’s compliance with plan of care and regularly communicate updates
to provider(s).
• Maintain compliance with regulations in the areas of disease prevention and treatment and
chronic diseases.
• Provide general nursing care to clinic patients when specialty providers are on site.
• Administer immunizations/vaccinations as necessary.
• Coordinate and conduct telemedicine visits with specialty providers as necessary.
• Performs all other job duties as assigned
